What is Hamstring Tendinopathy?
Hamstring tendinopathy is a load-related tendon condition, most commonly affecting the tendon where it attaches to the pelvis at the sit bone (proximal hamstring). Unlike an acute strain, this condition develops gradually. The tendon becomes sensitive when the demands placed on it exceed its current capacity. Typical features include a deep ache high in the back of the thigh or at the sit bone that builds during or after activity. Symptoms are often aggravated by running, sprinting, hill work, lunges, deadlifts, or long periods of sitting. Many people experience a cycle of temporary improvement followed by flare-ups when training intensity increases.

We consider shockwave therapy when hamstring pain has persisted for several weeks or months, when sitting discomfort remains high, or when attempts to increase training repeatedly trigger flare-ups.
The goal is to create a window where the tendon can tolerate progressive loading again. From there, treatment focuses on rebuilding capacity through staged strengthening, controlled exposure to running or sport, and clear progression targets.
